Job Description
STATEMENT OF THE JOB
The Psychiatric Rehabilitation Service (PRS) Provider will deliver psychiatric rehabilitation services in alignment with individualized treatment plans, within the context of our CCBHC model. The role requires collaboration with clinical teams to promote independent living skills, social and vocational functioning, and community integration for clients with serious mental illness. The provider will ensure compliance with NYS regulatory requirements specific to CCBHCs, contribute to service documentation and data reporting, and support the multidisciplinary team in meeting service and credentialing standards.RESPONSIBILITIES AND TYPICAL WORK ACTIVITIES
- Implement person‐centered psychiatric rehabilitation services (skills training, community integration, social/vocational support) consistent with individual treatment and rehabilitation plans.
- Facilitate group discussions or sessions ensuring active participation and respectful communication among all members.
- Ensure services are delivered in accordance with the nine-core service categories required for NYS CCBHCs.
- Maintain confidentiality and uphold ethical standards in all interactions.
- Participate in multidisciplinary team meetings and assist in developing and reviewing individualized recovery/treatment plans that align with CCBHC criteria: person centered, trauma informed, culturally competent, and recovery oriented.
- Work collaboratively with case management, peer support, outpatient therapy, crisis services, and other partners to ensure continuity of care for clients in compliance with CCBHC expectations for care coordination across systems.
- Maintain compliance with documentation, data collection and reporting protocols.
- Attend required training on evidence-based practices, trauma informed care, peer integration, and the CCBHC service model.
PREFERRED QUALIFICATIONS
- Must be a Certified Peer Specialist (CPS) in New York State or have bachelor's degree in human services field.
- Minimum one (1) year experience working with adults and/or children with MH/SUD needs.
- Understanding of recovery‐oriented and trauma‐informed approaches.
- Excellent interpersonal, communication, and organizational skills.
- Valid driver's license and reliable transportation.
- Demonstrated ability to work collaboratively in a multidisciplinary team and across systems.
Work Environment:
This is a professional, full-time (35 hours/week) position in an office in our Binghamton, Cortland or Oneida clinic so applicants from those areas are encouraged to apply. There may be travel and/or transportation of clients required to support service delivery.
