Job Summary We are seeking a passionate and dedicated Rehabilitation Specialist (PRP) to join our dynamic team. In this role, you will play a vital part in supporting individuals on their journey toward recovery and improved well-being. Your expertise will help clients regain independence, manage mental health challenges, and enhance their quality of life through personalized rehabilitation programs. Responsibilities Develop and implement comprehensive rehabilitation plans tailored to each client's unique needs, focusing on physical, emotional, and psychological recovery. Collaborate with multidisciplinary teams to coordinate care plans that include psychopharmacology considerations when appropriate. Monitor client progress regularly, adjusting treatment strategies to optimize outcomes and ensure continuous improvement. Educate clients and their families about coping strategies, medication management, and available community resources. Maintain detailed documentation of progress notes, and treatment plans in compliance with organizational standards. Experience Proven experience working as a Rehabilitation Specialist in a PRP program Familiarity with resources in the state of Maryland Experience create and implementing treatment plan goals Excellent communication skills with the ability to build trust and rapport with clients from diverse backgrounds. Ability to work independently as well as part of a multidisciplinary team dedicated to holistic care.
