Position Summary:
The Certified Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to provide rehabilitative services to residents, helping them regain independence in activities of daily living and functional mobility.
Key Responsibilities:
Provide direct patient care as outlined in the treatment plan developed by the Occupational Therapist Assist in the implementation of treatment goals and plans Monitor patient progress and report changes to the Occupational Therapist Accurately document treatments and patient responses using facility systems (e.g., Casamba, PointClickCare) Educate residents, families, and staff on therapy goals and home exercise programs Participate in team meetings, care planning, and discharge planning as needed Ensure compliance with federal, state, and facility regulations and policies
Qualifications:
Associate's degree from an accredited Occupational Therapy Assistant program Current COTA license in good standing with the State of California Experience in a skilled nursing or post-acute care setting preferred Strong communication and interpersonal skills Proficient in documentation software and electronic health records

Our CNAs play a vital role in providing compassionate, person-centered care in a transitional setting between hospital and home. General Purpose The Certified Nursing Assistant (CNA) provides routine daily nursing care and services to assigned residents in accordance with each resident's assessment, care plan, and facility policies and procedures. Essential Duties and Responsibilities Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, mobility, and dining Measure and record vital signs (TPRs), weights, and intake/output as instructed Respond promptly to call lights and ensure residents unable to call for assistance are checked frequently Observe and report changes in resident condition, skin integrity, or behavior to the Charge Nurse Assist with admissions, transfers, and discharges as directed Provide restorative and rehabilitative care under nursing supervision Maintain resident rooms, equipment, and personal belongings in a clean and orderly manner Follow infection control, safety, and confidentiality policies at all times Report accidents, incidents, concerns, or potential violations immediately Foster a warm, respectful, and positive environment for residents, families, and coworkers Supervisory Responsibilities This position has no supervisory responsibilities Qualifications Certificates, Licenses, Registrations Active California Certified Nursing Assistant (CNA) certification required CPR certification preferred Skills & Abilities Ability to read, write, and communicate effectively in English Ability to work in a fast-paced environment with compassion and professionalism Strong teamwork and interpersonal skills Physical Demands Ability to lift, push, pull, and/or move up to 25 pounds Frequent standing, walking, bending, and assisting with resident mobility May be required to assist during emergency or evacuation situations Work Environment Skilled Nursing Facility Noise level generally low to moderate Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions Equal Employment Opportunity (EEO) Statement Linwood Meadows Care Center is an Equal Opportunity Employer.
