Per Diem / Flexible (weekdays, weekends/holidays as needed)Job Summary:
We are seeking a dedicated and compassionate Occupational Therapist (OT) and/or Certified Occupational Therapy Assistant (COTA) to join our interdisciplinary team on a per diem basis. The OT/COTA will provide high-quality, goal-oriented therapy services to residents with a variety of diagnoses in our 80-bed facility. This role focuses on helping residents improve functional abilities, regain independence, and enhance quality of life.Responsibilities:
For Occupational Therapists (OT): Conduct initial assessments and develop individualized treatment plans. Deliver evidence-based occupational therapy interventions. Collaborate with nursing staff, physicians, and other therapy team members. Re-evaluate residents periodically and adjust care plans as needed. Document patient progress in accordance with facility and regulatory guidelines. Supervise and delegate appropriate tasks to COTAs in compliance with state regulations. For Certified Occupational Therapy Assistants (COTA): Implement treatment plans developed by the supervising OT. Assist residents with exercises, ADLs, and functional tasks to improve daily living skills. Monitor resident progress and report changes to the supervising OT. Accurately document treatment sessions and observations. Maintain a safe and supportive therapy environment.Qualifications:
For OT:
Current Occupational Therapy license in the state of practice. Graduate of an accredited OT program. Prior SNF or rehab experience preferred.For COTA:
Current COTA license in the state of practice. Graduate of an accredited OTA program. Experience in skilled nursing or rehab setting is a plus.Pay:
$30.00 - $45.00 per hourBenefits:
