Job Description

Certified Occupational Therapy Assistant (COTA) - Full-Time and/or
Part-Time Home Care Coverage Area:
Northwest & Northeast Philadelphia (Community-Based) Deer Meadows Home Health and Support Services is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our growing home care team. This community-based position offers the opportunity to help patients maintain independence and improve quality of life in the comfort of their own homes.
Responsibilities:
Provide occupational therapy treatments under the direction of a licensed Occupational Therapist Assist patients with ADLs, functional mobility, strength, coordination, and safety awareness Educate patients and caregivers on adaptive techniques and home safety Complete timely and accurate documentation using Home Care Home Base (HCHB) Communicate effectively with the interdisciplinary care team Monitor and report patient progress
Qualifications:
Graduate of an accredited Occupational Therapy Assistant program Current Pennsylvania COTA license Valid driver's license and reliable transportation Home health experience preferred, but not required Experience with Home Care Home Base a plus Strong communication and organizational skills
