Administers specified evaluations and provides or assists in the provision of specified treatment/activity protocols/programs for rehabilitative therapy patients under the guidance and supervision of a licensed therapist and in accordance with the physician's plan of care. Monitors patient progress and modifies treatment plans as per the licensed therapist's instructions. Reports to the assigned Physical or Occupational Therapist and Clinical Supervisor.Responsibilities:
Evaluates and reviews pertinent data relating to overall treatment planning and evaluation; administers standard evaluations and elicits information regarding patients' existing conditions, limitations, home environment and capacities; in conjunction with the licensed therapist, identifies appropriate treatment goals and the type of remedial care to be administered. Administers appropriate treatment/activity programs, including specialized treatment, as specified or directed by the licensed therapist and in accordance with plan of care and established policy and procedures; monitors patient progress and makes recommendations concerning treatment options to the licensed therapist; modifies treatment plans as approved. Participates with the licensed therapist in recording/documenting appropriate clinical and progress notes in patients' medical records including patient's response to treatment plan and progress towards goals; ensures transmittal of progress notes to physician when patient has appointment. Coordinates therapy activities with physicians, therapists, nurses, CNAs and other Home Health therapy staff; communicates appropriate patient information and/or patient changes to the treatment team and provides necessary input for patient care, referrals and discharge planning purposes. Participates in educating the client and family about treatment plans, goals, priorities, expected outcomes, use of equipment and/or devices, etc. as appropriate; assesses patient/family response to treatment protocols. Maintains therapy equipment and ensures adequate inventory of departmental supplies. Is supervised by the licensed therapist no less than every 30 days. Local travel required. Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested. Performs other duties as assigned.Qualifications:
Minimum Education:
Associate's degree (or equivalent) from an accredited program for Physical Therapy Assistants or Occupational Therapy Assistants.Minimum Experience:
Two (2) years' experience as a licensed rehabilitation therapy assistant (i.e. LPTA or COTA) in a health care environment; previous home health experience preferred.Licensure Requirement:
Must have and maintain Tennessee State licensure as a Licensed Physical Therapist Assistant (LPTA) or Certified Occupational Therapy Assistant (COTA). Employee must have a valid Tennessee driver's license Class D and state mandated minimum insurance coverage.
