PRN Occupational Therapy Assistant (COTA) - In-House Rehab | Hamilton, TX Focused Post Acute Care Partners (FPACP) is a dynamic company with an expectation of excellence in the delivery of long-term care to every patient every day. Position Summary Under the supervision of a licensed Occupational Therapist, you will provide skilled occupational therapy services to residents, implement individualized treatment plans, and support patients in achieving their highest level of independence and quality of life. Key Responsibilities Deliver therapeutic interventions to residents with short-term and long-term care needs Provide skilled OT services in accordance with physician orders and plan of care Assist residents with activities of daily living (ADLs) and functional mobility Educate patients, families, and caregivers on treatment goals and progress Document treatments, patient responses, and changes in condition Collaborate with the interdisciplinary team on care planning Maintain compliance with company policies and regulatory standards Qualifications Active Texas COTA license Certified Occupational Therapy Assistant (COTA) credential Good standing with all licensing boards and regulatory agencies Experience in long-term care, skilled nursing, or geriatric rehabilitation preferred Familiarity with Net Health documentation system a plus Strong communication, organization, and teamwork skills Why Join Us?
