JOB SUMMARY
Provides occupational therapy treatments as directed by a qualified Occupational Therapist to patients in the facility, communicates with physicians, and coordinates care with other team members. This job is doing home health occupational therapy.
RESPONSIBILITIES
1. Provides timely care when assigned to a case. Function within parameters of your knowledge and experience, practicing safely and competently and comply with professional standards of practice and care which you have been trained and are appropriate for your skill level for which you have had experience. 2. Treat patients according to the treatment plan approved by the physician following the care plan developed by the supervising OT. 3. To notify the case manager/OT of all changes in patient's condition whether an increase or decrease regarding progress, problems and plans. 4. Insure that supervisory visit(s) is preformed by the supervising OT (OT evaluates the effectiveness of the services furnished by the assistant) whenever the patient's condition improves, declines and/or requires a change in the plan of care. Performs services planned, delegated and supervised by the OT. Discharges must also be supervised and completed by the supervising OT. 5. Documentation will show communication and supervision between the therapist and the assistant. The communication will show the patient condition, response to therapy and the need if any to change the POC. 6. Participate in in-service training programs for and with Agency staff and participate in staff meetings as required. 7. Prepares clinical notes, communicates with OT, and participates in educating the patient and family. 8. Communicates with supervisors, other health team members and the physician regarding patient progress, problems and plans. 9. To consistently function as a member of the team and Agency in a positive manner. 10. To respect the Agency Patients Bill of Rights and Responsibilities. To maintain confidentiality of patient information. 11. To submit accurate and complete time reports to the office in a timely manner. 12. To honor your commitment to work assignments that you accept. 13. To comply with Agency Employee Policies and Procedures. 14. To seek supervisory guidance whenever you have a question as to a specific task or responsibility. 15. To accurately and safely use equipment and supplies appropriate and necessary for care of patients 16. To consistently use proper body mechanics and use proper infection control techniques and to maintain environmental safety for your patient(s). 17. To maintain a friendly, caring attitude, seeking to assist your patient with his/her needs while following the POC. 18. To use your skills and individuality to communicate to management where operations/patient care can be improved. 19. Comply with all applicable regulations; including but not limited to proving accurate documentation of patient activity, operate under the instructions of a physician, provide only medically necessary services billed to a third party, meet all other applicable reimbursement criteria, protect the patients' health care information and report any potential fraud and abuse to the compliance officer.
