Job Description

Transitions At Home, is currently seeking to hire a motivated, reliable, Occupational Therapist Assistant (COTA) who will make a meaningful difference by helping patients regain independence in the comfort of their own homes and values one-on-one care, flexible scheduling, and being part of a supportive, collaborative healthcare team. What does a Transitions at Home Occupational Therapist Assistant do? An Occupational Therapist Assistant (COTA) provides occupational therapy services to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Occupational Therapist.
Hours:
Part-Time 20-29 hours per week 8:00 am

Job Requirements:
Active Wisconsin Occupational Therapist Assistant License Two (2) years' experience preferred. CPR Certification A valid Wisconsin Driver's License and Car Insurance Transitions At Home, a non-profit home health organization serving Central Wisconsin, provides quality care, safety, and well-being in each patient's home.
