Job Description

Job Summary Under general supervision, provides primary care and or advanced life support, including medical evaluation, treatment, and stabilization of the critically ill and injured with the goal of reducing morbidity. Supports existing health services; provides mobile integrated health services in partnership with other health professionals; extends access to health services delivery in underserved and general populations, including, but not limted to, primary care, public health, disease management, prevention and wellness, and mental health. Exercises appropriate judgement in all situations. Strives to ensure prompt response to emergency calls when required; and performs other duties as assigned. Essential Functions All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference. Performs all primary job responsibilities required for a Paramedic and responds to emergent calls when requested. Performs POC testing with approval of Administration Provides in-home and community-based patient assessments, treatments, and follow-up care. Supports chronic disease management, medication compliance, and patient education to improve long-term health outcomes. Conducts post-hospital discharge visits to reduce readmissions and emergency department use. Collaborates with physicians, nurses, social workers, and other healthcare providers to coordinate patient care. Identify and connect patients with appropriate healthcare, social, and community resources. Documents patient encounters accurately and maintain compliance with organizational and regulatory standards. Promotes health, wellness, and prevention through patient and community education. Responds to urgent but non-emergent patient needs, escalating to emergency services when necessary. Maintains equipment, station, vehicles, and supplies in ready state for community response. Performs other duties as assigned. ​ Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Requirements Education•High School diploma or equivalent OR post-high school diploma/highest degree earned preferred. Completion of approved Paramedic Education Program. Experience•Five (5) years paramedic experience. In Lieu Of In lieu of the education and experience requirements noted above, completion of an approved Paramedic training program and two (2) years functioning as a Community Paramedic may be considered. ​ Required Certifications, Registrations, Licenses
SC PARAMEDIC
•AEMT A valid driver's license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart. Community Paramedic Certification (must be enrolled in a SC approved CP Training Program and/or obtain Certification within six (6) months of hire). International Board of Specialty Certification (IBSC) for Community Paramedic•Certified (CP-C) within twelve (12) months of hire. ​ Knowledge, Skills and Abilities Knowledge of office equipment (fax/copier) Proficient computer skills (word processing, spreadsheets, database, data entry) Ability to attend required training to maintain current certification and licenses.
