ISNP Care Navigator/LPN Part-Time Associated Care Ventures Inc - 3.0 Birmingham, AL Job Details Full-time From $25 an hour 2 hours ago Qualifications Customer communication Electronic health records (EHR) management Microsoft Office LPN Productivity software Full Job Description The Care Navigator plays a crucial role in supporting members by navigating healthcare services and resources to improve health outcomes and enhance member experience. This position serves as a liaison between the health plan, members, and healthcare providers, ensuring that members receive appropriate care, understand their benefits, and access necessary services.
Major Duties and Responsibilities:
Member Support & Outreach:
Proactively engage with members, especially those with complex medical or behavioral health needs, to access healthcare needs, explain benefits, and help navigate the healthcare system.
Care Coordination:
Collaborate with care coordinators, providers, and other healthcare professionals to coordinate medical and social services, including transportation, prescription refills, and follow-up care.
Education & Guidance:
Educate members on preventive care, wellness programs, and self-management of chronic conditions to promote healthy behaviors and improve quality of life.
Resource Identification:
Identify and connect members with community resources, support services, and health plan program (e.g. wellness, disease management) to address social determinants of health.
Documentation & Reporting:
Maintain accurate and timely documentation of health risk assessments, member interactions, care plans, and foll-up activities in the care management system.
Data Collection & Analysis:
Track and report key performance metrics, identifying trends and opportunities to improve care delivery and member satisfaction.
Compliance:
Ensure all activities adhere to health plan policies, HIPPA regulations, and applicable state and federal guidelines.
Education and Experience requirments:
LPN with experience Specialized Skills/Knowledge Excellent communication and interpersonal skills, with a patient-centered approach. Strong organizational skills and the ability to manage multiple tasks and prioritize effectively. Proficient in EHR and case management systems, along with basic Microsoft Office skills. Certification in case management or case management experience preferred. Location Requires heavy travling to the North Alabama area Compensation This is a part-time position and does not offer full time employee benefits.
