Job Description
About the
Role:
The Community Liaison will serve as a key connection between our home health and hospice team and local healthcare providers, facilities, referral sources, patients, and families. The ideal candidate is confident in networking, experienced in healthcare marketing, and able to develop strong community relationships that support organizational growth and patient care services.Key Responsibilities:
- Develop and maintain relationships with hospitals, physicians, nursing facilities, assisted living communities, and other referral sources
- Promote hospice services within the Lake Havasu community through outreach and education
- Identify opportunities to grow census and strengthen community presence
- Coordinate and participate in marketing events, meetings, and community activities
- Collaborate closely with clinical and leadership teams to support patient admissions and continuity of care
- Maintain accurate records of referral activities and community interactions
- Assist in developing marketing strategies and outreach initiatives
- Represent the organization professionally and compassionately at all times
- Support organizational goals through effective communication and relationship management
Qualifications :
- Bachelor's degree in Marketing, Business Administration, Healthcare Administration, or related field preferred
- Minimum of three (3) years of healthcare marketing or hospice/community liaison experience preferred
- Strong communication, negotiation, and public relations skills
- Ability to build and maintain professional relationships within the healthcare community
- Excellent organizational skills with the ability to work independently and collaboratively
- Knowledge of hospice services and healthcare operations preferred
- Self-motivated, flexible, and goal-oriented professional
- Valid driver's license, reliable transportation, and current auto insurance required
