Full-Time, Employed Mon- Fri, 9:00am

Position Summary:
Under the Agency's policies and professional requirements, participates as a member of an interdisciplinary clinical team working with individuals diagnosed with General Mental Health diagnosis and or substance use disorder, or Serious Emotional Disturbance. Assists patients with identifying treatment and recovery goals and provides supportive services to help patients reach their goals. Provides recovery-based coaching for patients and their families related to their clinical recovery goals. Assists members in pursuing and achieving their educational and social goals through direct one-to-one and group psychosocial support, brief clinical interventions, motivational interviewing, active listening, and sharing their personal shared experience to support the patient's recovery. Works under the direct supervision of the Children's Clinical Site Manager.Duties May Include:
Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patients, their families, behavioral health, medical and dental providers, community resources, and others in ensuring that all services prescribed in the individualized service plan are implemented. Active participate in the Child and Family Team meeting. Conduct face-to-face interactions with patients, family, or other involved parties in the home, community, and recovery clinic for the purpose of assessing, maintaining, or enhancing a person's functioning. Participate in Consumer Advisory Council at the respective site. Promote patient and family/natural support participation in Advisory Council meetings. Facilitate peer support and psychosocial groups, either in a clinic or community setting, and engage patients in group participation as related to their individual recovery goals. Provide transportation for the patients as appropriate and as medically necessary, and determined by the clinical team, in a Terros Health provided vehicle. Coordinate alternative transportation for patients and identified natural supports, as medically necessary. Provide excellent customer service to patients, internal and external provider agencies, state entities, or other stakeholders (e.g., probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.) as needed. Provide direct assistance in finding necessary resources other than covered services to meet basic needs for patients served. Encourage, assist, and support patients in expressing their perspective throughout the processes of assessment, service planning, and service delivery, while maintaining a focus on their recovery. Conduct outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place. Respond to patient crises in the field. Collaborates with CFT and other Clinical Team members.
