Job Description

Job Description An employer is looking for a remote Patient Outreach Specialist for a healthcare start up. This role serves as the primary point of contact for patients and caregivers, helping guide them through enrollment into a healthcare program focused on supporting individuals with chronic conditions. This role is responsible for delivering a high-quality, patient-first experience while driving engagement, education, and program participation. Key Responsibilities
  • Contact patients and caregivers to introduce the program and guide them through the consent and enrollment process.
  • Clearly communicate program benefits and services to patients living with chronic conditions in a way that is informative and approachable
  • Execute structured outreach workflows with strong attention to detail and accuracy, while identifying opportunities for process improvement
  • Own and achieve key performance metrics, including consent rates, scheduled visit rates, and no-show recovery
  • Maintain a warm, professional, and patient-friendly tone across all interactions
  • Partner cross-functionally with internal teams, including technology, to provide feedback on outreach and scheduling tools and contribute to process enhancements Desired Traits

Salary:
$65,000
