Organization:
SSRRH-Sutter Santa Rosa Reg Hospital Position Overview:
Helps patients access evidence-based treatment for substance use disorders (SUDs) with a focus on low-threshold access to medication for addiction treatment (MAT) and harm reduction. Conducts outreach and identification of patients, conducts initial brief assessments, introduces patients to MAT programs and services, facilitates appointments at outpatient MAT clinics, serves as a resource and coach for their clients, and advocates for culture change in the hospital.

EDUCATION
High School or General Education Diploma (GED)
CERTIFICATION & LICENSURE
Community Health Worker certification within 18 months of hire
SKILLS AND KNOWLEDGE
: Possesses a nonjudgmental, energetic, positive, harm-reduction approach to assisting patients with SUD. Interest/proficiency in working with individuals recently released from incarceration, homeless individuals, and other marginalized populations. Understanding of SUD as a medical condition and MAT as an effective, evidence-based treatment. Ability to communicate with patients clearly, respectfully, and in a culturally appropriate manner. Respect for patient confidentiality and privacy. Ability to use a computer and to learn to use electronic health records.
