Full Job Description Statement of Purpose:
This position is responsible for providing outreach and health screening services to patients who have limited access to medical care by facilitating an increase in quality, availability, and effectiveness of educational and community-based programs designed to prevent disease and improve quality of life. Will educate the community about Sexually Transmitted Infections/Diseases (STI's/STDs) and screening services offered. Perform mobile rapid syphilis testing in Tulare County. Attend all ALTURA events to promote STD testing, treatment, and educate the community about ALTURA clinic services. Essential Functions, Knowledge, Skills and Abilities Educate clients about STDs, prevention and treatment Educate clients about the importance of using safe sex materials (condoms and lubrication) to prevent STIs and STDs. Become certified to administer rapid syphilis testing Collaborate with The Source, a LGBTQ+ community based organization, to collaborate and facilitate STD testing events. Responsible for STD Collaboration Grant patient data reporting after The Source Testing events. Schedule and coordinate outreach events throughout the community. Serves as a liaison for the health team, patient and family. Promote increased community awareness of all services provided at Altura Centers for Health. Enhance education for patients, families and communities to support the use of contraceptives to reduce unplanned pregnancy. Provide health education relating to teen pregnancy, STD and adolescent health, mental health, substance abuse and all health prevention education. Coordinates and participates all Altura Outreach Events and health fairs, and other community projects with the goal of increasing patient population, increasing awareness about patient programs and clinic services. Coordinates and Participates in community medical screenings; take patient vitals (temperature, blood pressure, height, and weight) and perform glucose and hemoglobin screenings. Assist with dental screenings as needed. Support the promotora program. Will assist patient in outreach and education on the Affordable Care Act Health Exchange "Covered California" health insurance plans, Medicaid expansion or CHIP. Will follow all the standards and requirements from Covered California and State Medicaid. Serves as a liaison for the health team, patient and family. Serves as a patient advocate providing support and a voice for patients who cannot effectively do this on their own. Will provide case management (non-clinical) to patients to ensure they are following through with their prescribed treatment plan. Advises and makes recommendations to physicians, patients and families regarding the availability of resources and coverage, locates the resources, and arranges for the provision of cost effective services for patients. Assesses patient's resource status and seeks out alternatives when coverage is not available. Ensures referrals to the county eligibility worker for patients that are eligible for State sponsored programs. Works closely with the health educator to provide education regarding disease processes, disease prevention and treatment. Maintains a record of patients served that includes all support services and efforts made to resolve issues of care. Provides presentations on ALTURA to church groups, local community groups, and any service organizations to educate the community on ALTURA services. Attends and collaborates with local collaborative, consortia, special interest groups, etc. to help maintain resources and useful information for our patients. Uses positive and appropriate telephone etiquette/techniques when communicating over any phone. Ensures that communication with staff, patients and external contacts occur with courtesy, tact and a positive attitude. Participates in relevant staff training such as orientation and staff development meetings. Actively participates in community collaborative. Effectively communicates verbally with patients. Able to evaluate patient's response to care through observation. Professionally handles patient complaints. Accurately completes patient information for patient programs Assists patient in filling out required documents for patient programs. Ensures a timely response of information for the prescription assistance program. Meets all attendance and punctuality requirements to ensure proper coverage and quality service. Professional and appropriate dress as required by the position and compliant to the ALTURA dress code. Demonstrates an ability to resolve interpersonal and professional conflicts appropriately. Participates in staff meetings, trainings and committees. Keeps all matters related to the organization confidential in compliance with confidentiality policy. Takes pride in job performance as evident in compliance with job responsibilities. Assumes responsibility for work performance and is able to be self-directed. Develops trust and teamwork with staff and patients. Performs in a professional manner and puts forth their best effort. Communicates openly and consistently with Supervisor(s) and all staff. Maintains strictest confidentiality. Assumes other duties as assigned.
Required Education and Experience Minimum Education:
High school graduate of GED equivalent required. One year experience in Ambulatory Care setting. Bilingual in English and Spanish is required. Graduation from an accredited program for Medical Assistants and proof of certification is required. Valid California Driver's License and insurable through Altura policy Other Duties Maintains a positive and respectful attitude while delivering excellent "customer service". Work Environment Office environment with controlled temperature. Outdoor environment with outdoor weather conditions. Position Type/Expected Hours of Work This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.

Travel is expected for this position. A typical example of travel would be driving to Altura's various clinic locations, events, or trainings.
