Responsibilities:
Complete child development screening tools with patients during visits or over the phone. Explain the importance of monitoring child's development and how screenings will be used to evaluate a child's developmental progress and identify any concerns. Ensure screening tools are appropriately documented in the patient's electronic health record. Work collaboratively with pediatric care team to identify missing care gaps (such as immunizations) and Parent Educator/Case Manager to support patients and families with needed referrals. Conduct and score child development screening assessments during Well Child Visits Work with patients to understand the importance of child development screening reinforcing the importance that the parent/caregiver is the child's first and best early-life teacher Educate patients on child development milestones Encourage clients to attend appointments, program classes and follow up with referrals Share any child development concerns patients report with Clinician and participate in care team meetings Review schedule and patient charts to prepare for the day Promote the integration of services including behavioral health care, dental, etc and provide community resources and other appropriate services Work with other departments to ensure seamless care and provide resources
Qualifications:
Minimum Qualifications High school graduate or equivalent Bilingual English/Spanish Valid CA driver's license and vehicle insurance, reliable transportation; Preferred Qualifications Associate's degree in related field with two years' care coordination or health education experience Clinical or medical office setting experience Resourceful community liaison experience navigating through complex health systems Required Skills/Knowledge/Abilities Ability to work flexible hours. Ability to present information effectively both verbally and in writing to individuals and groups from various cultural, ethnic and educational backgrounds Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities Pay Range $22.00 - $24.00
