Join Our Team at Valley-Wide Health Systems, Inc. We are currently seeking Patient Care Navigator to join our team, where you will play a key role in assisting patients with navigating the healthcare system, overcoming financial and social barriers, and improving health outcomes. Key Responsibilities:
Patient Assistance & Program Enrollment Assist patients in resolving issues related to healthcare access, financial barriers, and social needs. Help patients complete enrollment applications for programs such as Senior Dental, Medicaid, Sliding Fee Discount Program, and MedRide. Request interpreters, transportation services, or prescription assistance as needed. Verify and collect documentation to support eligibility for state or local assistance programs. Maintain confidentiality of all patient information. Health Navigation & Referral Support Conduct comprehensive assessments for new and referred patients, evaluating clinical and psychosocial needs. Guide patients in understanding healthcare recommendations, treatment plans, and available services. Assist in scheduling specialty care, imaging, and diagnostic appointments, ensuring patients are informed about authorizations and requirements. Follow up on pending and denied authorizations, appealing as necessary. Support internal and external referrals, ensuring continuity of care. Monitor patient treatment plans, provide assistance in understanding medical benefits, and coordinate communication with insurance providers. Team Collaboration & Documentation Work in a medical home model, helping patients establish a Primary Care Provider and participate in healthcare planning. Participate in team huddles and collaborate with interdisciplinary healthcare teams. Document all patient interactions, social determinants of health screenings, referrals, and case management activities in the Electronic Health Record (EHR) system. Assist in running reports and supporting quality improvement initiatives by scheduling preventive and routine exams. Provide additional clinic support, including managing patient portal requests, scheduling follow-ups, and maintaining records for audits.
Qualifications & Skills:
Education:
High school diploma required; Bachelor's degree in Business, Healthcare Administration, Human Services, Social Work, or a related field preferred OR one (1) year of qualifying experience.
Experience:
1+ year of experience in customer service, support, coordination, or case management.
Skills:
Strong critical thinking and problem-solving abilities. Proficiency in Microsoft Word, Excel, and Outlook. Ability to communicate professionally in person and over the phone. Familiarity with community resources and assistance programs. Sensitivity to low-income and diverse populations. Bilingual Spanish-speaking preferred but not required.
Differential Pay:
This position includes a $2.50 per hour differential for all hours worked. Why Join Valley-Wide?
