See it for yourself at Elimpark.org Position Summary The Support Services Coordinator supports residents of Elim Park Place (EPP) during periods of transition and life change. This role focuses on proactive engagement, coordination of resources, and ongoing relationship-building to promote emotional well-being, successful adjustment, and continuity of care. The Support Services Navigator provides person-centered care and is involved prior to move-in and remains a consistent point of support throughout a resident's life in EPP. Engage with future residents/family to support a smooth transition to our retirement community. Key Responsibilities Resident Transition & Support Engage with prospective residents prior to moving-in to identify needs, provide resources, and support a smooth transition to EPP. Meets with prospect and family at time of deposit/renovation meeting. Maintains open communication through day of moving in. Complete initial resident intake forms and ensure accurate, timely documentation. Develop schedule of support as needed for each resident moving in which would include a minimum check in 24 hours following move in, one week following move in, and 30 days following move in. Meet with every EPP resident, who is not on ALSA services, no less than once every six months to assess well-being, identify emerging needs, and provide support. Develop, implement, and monitor follow-up procedures for residents experiencing significant life changes, including but not limited to: Death of a spouse or loved one Loss of a child Revocation of driving privileges Assuming caregiver roles Other major emotional, social, or functional changes Care Coordination & Collaboration Review and share relevant resident information with appropriate internal and external partners, ensuring confidentiality and person-centered care. Navigator will comply with all rights and responsibilities required by HIPAA regulations. Partner with Assisted Living Services, Life Enrichment, ALSA, outpatient therapies, and other healthcare services to help residents access needed support. Work closely with Life Plan Specialists, Elim Park Rehabilitation department and resident welcome committee to ensure the move-in experience is welcoming and informed. Navigator is responsible for greeting residents at time of move in. Provide them with a tour and introductions. Provide key contact information. And develop an individualized follow up and onboarding plan. Meet regularly with the Director of Assisted Living Services to review resident needs, transitions, and ongoing support plans. Monthly meetings with support partners CMHS, HHC, and PACT to discuss available services and any resident issues. Participate in care plan meetings for EPP residents in the Health Care Center. Assist with supporting discharge plan. Daily review of first responder log to review any falls or incidents. Ongoing assessment of current processes to evaluate effectiveness and efficiency. Also, recommend and develop additional processes as needed. Communication & Advocacy Serve as a consistent, trusted point of contact for residents navigating change. Advocate for resident needs while promoting independence, dignity, and choice. Support interdisciplinary communication to enhance resident outcomes and satisfaction. Promotes the services through pop up events, attendance at Resident President Meeting and other formats available throughout EPP. Qualifications Bachelor's degree in social work, psychology, human services, healthcare, or a related field preferred. Experience working with older adults, transitional care, or supportive services are strongly preferred. Strong interpersonal, organizational, and communication skills. Ability to work collaboratively across departments and with external partners. Demonstrated compassion, discretion, and ability to support individuals during emotionally sensitive times.
