Job Description

As a(an) Patient Care Navigator with HCA Florida Bayonet Point Hospital you can be a part of an organization that is devoted to giving back! Job Summary and Qualifications The Patient Navigator serves as the primary clinical and operational coordinator for patients undergoing structural heart procedures, including transcatheter aortic valve replacement (TAVR), MitraClip, left atrial appendage closure, and other advanced cardiac interventions. This role facilitates seamless patient care across the continuum, from referral and evaluation through procedure, recovery, and follow-up. What you will do in this role:
  • Coordinate patient evaluations, diagnostic testing, and multidisciplinary heart team consultations.
  • Serve as a liaison among physicians, advanced practice providers, nursing staff, and ancillary departments.
  • Educate patients and families regarding procedures, treatment plans, and post-procedure care.
  • Maintain regulatory and accreditation requirements related to structural heart programs.
  • Collect, manage, and submit clinical data to national registries and quality databases.
  • Monitor outcomes and support quality improvement initiatives.
  • Coordinate scheduling for procedures and follow-up appointments.
  • Participate in program development, community outreach, and physician referral activities What qualifications you will need: Associate degree in health care field; Bachelor's degree preferred.
Minimum 1-year clinical specialty area experience required. Currently Florida State Registered Nurse license. Current BLS (Basic Life Support) Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills.
