Job Description

Job Summary Jessie Trice Community Health System is seeking a dedicated and compassionate PrEP Navigator to join our healthcare team. The ideal candidate will possess a strong understanding of HIV prevention strategies, social work, and case management, with a focus on supporting individuals at risk for HIV. As a PrEP Navigator, you will play a vital role in guiding patients through the process of accessing and adhering to pre-exposure prophylaxis (PrEP), ensuring they receive comprehensive care and education. This position offers an opportunity to make a meaningful impact on community health by promoting HIV prevention and reducing transmission rates.
Responsibilities :
PrEP Navigator will focus on carrying out program activities to meet contractual agreement set by HRSA on behalf and for the clients; Successfully engage and retain in care HIV-positive and HIV-negative individuals in medical care, develop, implement, and monitor agreed upon service plan for each client and assist clients in accessing needed services. The position focuses on providing knowledge and skills regarding HIV/AIDS, STDs and addressing the social determinants of health and community barriers that increase health disparities for LGBTQ+ and underserved community members. This position is to create and promote a healthier, more positive environment for LGBTQ+ and underserved community members. HIV/STD testing for the Client Service Department, as well as attending health fairs as requested and working non -traditional hours.
Details of Education and Skills:
High school diploma or some college with minimum two years related experience and/ or training . Excellent Communication skills ( oral & written ) Must be proficient with Microsoft office suite. Must be able to work individually/team, able to work with the LGBT community
Special Requirement:
CPR certificate HIV 104 Training Certificate, HIV 501
Training Certificate Pay:
$45,000.00 per year
