Job Description

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and highly skilled Dementia Care Navigator to join our team in Broward County. This full-time position works Monday through Friday, 8:30 a.m. - 5:00 p.m . The Dementia Care Navigator provides essential assessment, education, and support to individuals living with dementia and their caregivers, strengthening our mission to guide families through meaningful transitions. Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness. What You'll Do Conduct psychosocial assessments for people living with dementia and their caregivers as a member of the Dementia Education Program team. Provide Care Navigation services, including initial GUIDE screenings/assessments and ongoing monitoring and support for patients enrolled in the GUIDE program. Assist in the development and implementation of the social services component of the care plan. Join a team built on kindness, compassion, and Full Life Care for All. What You'll Need Degree in Social Work. Valid Florida driver's license , reliable automobile, and current auto insurance. Minimum of (1) one year experience as a caseworker with an emphasis on counseling, crisis management, community agency referral and grief support, preferred. Experience in developing collaborative relationships with referral sources and first responders. Ability to write routine reports and correspondence. Expertise and knowledge regarding older adults, dementia, dementia care and support, family and informal caregiver needs and challenging behaviors, preferred. Skills and experience in facilitating small group learning and discussion programs; including support groups. Ability to operate a facsimile, copy machines, computers and other common office machines, with working knowledge of MS Word, Outlook, Excel and other software appropriate to positions.
