Job Summary We are seeking a dedicated and knowledgeable RN Navigator Perinatal to guide expectant mothers through their pregnancy journey, ensuring seamless coordination of care from prenatal to postpartum stages. The ideal candidate will possess extensive clinical expertise in maternal and neonatal health, along with exceptional communication skills. This role involves providing personalized support, facilitating discharge planning, and collaborating with multidisciplinary teams to optimize patient outcomes. A $3,000 sign-on bonus is available for qualified applicants. Duties Serve as a primary point of contact for perinatal patients, offering education and emotional support throughout pregnancy and postpartum periods. Coordinate multidisciplinary care plans by collaborating with obstetricians, pediatricians, social workers, and other healthcare professionals. Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations relevant to pregnancy and neonatal health. Facilitate diagnostic evaluations and manage case documentation within medical records systems while ensuring compliance with HIPAA regulations. Assist with discharge planning by organizing post-acute care services, outpatient follow-ups, and community resources tailored to individual needs. Monitor patient progress through continuous assessment of symptoms, vital signs, and response to treatment interventions. Educate patients on pregnancy-related topics such as prenatal care, labor preparation, postpartum recovery, and infant care practices. Skills Extensive clinical experience in critical care settings such as ICU or Level II/Level I Trauma Centers with exposure to obstetric emergencies or neonatal intensive care units (PICU). Strong knowledge of hospital medicine, emergency medicine protocols, infectious disease management, and acute care procedures. Proficiency in medical terminology, medical records management, discharge planning processes, and diagnostic evaluation techniques. Demonstrated ability in patient assessment, intake procedures, utilization management, case management, and post-acute outpatient services. Familiarity with Geriatrics, behavioral health considerations in perinatal populations, and physiology knowledge relevant to maternal-fetal health. Excellent communication skills coupled with a compassionate approach to patient service and education. Ability to navigate hospital experience environments effectively while maintaining compliance with HIPAA standards. Pay:
$31.53 - $52.24 per hour
