As a Health Care Navigator - Pensacola , you'll empower Veterans and their families to overcome barriers to healthcare, stability, and housing through compassionate support and expert guidance. Job Summary Provide healthcare navigation and related supportive services to homeless and at-risk Priority Veteran clients. Responsibilities Collaborate with Case Management team to develop individualized service plans to assist homeless or at-risk Veterans and their household members in overcoming health and disability related challenges. Provide support for referred Veterans/families with specialized health and/or disability needs, including mental health and substance Complete tailored assessments and care plans informed by Veteran's/family's specific situation and their goals for long-term housing stability Meet weekly with Director of VC to staff cases and provide crisis intervention/support to Case Management team as needed Assist referred Veterans/families in accessing healthcare systems including but not limited to, gaining entry to VA healthcare or community care, facilitating enrollment into the VA Medical Center, gathering documentation and complete paperwork required for enrollment, following up on enrollment progress Complete SOAR training and obtain SOAR certification to assist Veterans/families in applying for
SSDI/SSI
benefits through the Social Security Administration Assist Veterans/families with processes related to VA Disability benefits and discharge status, including appeals process and/or obtaining related paperwork Help Veterans/families get access to appointments when needed by supporting veterans in identifying health care needs, and working collaboratively with heath care teams to facilitate access to care Assist Veterans/families in utilizing available services including preventative health care Help Veterans/families identify and modify barriers to adhering to recommended health care plans, maintaining housing stability and improving overall functioning Provide client-centered advocacy Assist Veterans/families in understanding and communicating with providers to make informed decisions about health care Provide wellness education to Veterans/families Maintain accurate and timely case documents in accordance with SSVF grant guidelines Actively participate in professional development training Actively participate in weekly staff meetings and individual supervision Qualifications Knowledge & Skills Some knowledge of healthcare systems and/or veteran and military family programs Understanding of disabilities, including Serious Mental Illness (SMI) and substance use disorder Case management experience, including knowledge of trauma-informed care and person-first practices Excellent interpersonal and communication skills Excellent computer skills to include Microsoft Office suite and Client Management System Able to work as team member Able to establish rapport with clients who are homeless or facing homelessnes Familiarity with Housing First, Veteran's Administration (VA) and/or US Military a plus Education/ Experience Bachelor's degree in social work, healthcare or similar field required Master's degree in social work preferred Military experience preferred 2-3 years' experience required Working Conditions Services are provided in office, in the community, and in client homes Occasional afterhours/weekend work Travel required Requirements Must meet and maintain eligibility requirements to be insured by UWCA's insurance Valid Alabama driver's license, reliable transportation and proof of automobile insurance commensurate with State law Cell phone Background checks to include criminal record, drug screen & TB testing Ability to lift 30 lbs.
