POSITION-SPECIFIC JOB RESPONSIBILITIES
Supports patient care coordination and value-based care initiatives by identifying preventative and chronic care gaps, conducting patient outreach, and assisting patients with access to healthcare resources. Uses EPIC and Care Message platforms to manage outreach campaigns, document communication, and coordinate follow-up care. Collaborates with care teams, insurance providers, and outside agencies to improve patient outcomes and maximize quality and reimbursement goals. Coordinates pre-operative appointments, assists with administrative tasks and special projects, and supports training initiatives as needed. Maintains confidentiality, provides excellent customer service, participates in quality improvement activities, and follows all safety, attendance, and organizational standards.
Experience/Education :
