Job Description

Salary range for this role is $17-18.50/hr.
POSITION-SPECIFIC JOB RESPONSIBILITIES
Achieves Value Based Contracts Financial and Quality goals by monitoring patient outcomes, completing patient outreach for preventative care and screenings, chronic condition follow up, etc.•Assists with special projects, including training and support for other departments Assists with administrative functions as needed. Use EPIC to identify patients that are due for preventative and chronic condition care gaps. Conducts patient outreach through phone calls, letters, etc. to inform patients of needed care. Communication is documented accurately within the chart. Works with patients to identify barriers to care and helps identify resources to address barriers (e.g. transportation, etc.). Maintains clear and open lines of communication with the care teams. Collaborates with outside agencies/payer sources to close care gaps. Works with manager to establish outreach goals to then collects data and monitors the progress.
Experience/Education :
