Overview UnityPoint Clinic Community Health Worker East Des Moines Family Medicine Monday-Friday 8:30AM-5:00PM Full-time + benefits We are seeking a compassionate and resourceful Community Health Worker (CHW) to join our busy Family Medicine team in East Des Moines. In this vital role, you will work closely with patients, providers, resident physicians, and care team members to identify barriers to care and connect patients with community resources that support their overall health and well-being. Serving a diverse patient population, the CHW provides education, referrals, advocacy, and follow-up services to help patients navigate healthcare and community systems, improve access to resources, and achieve optimal health outcomes.   • Build positive relationships with patients, families, providers, and care team members.
  • Assess patient needs and connect individuals and families with appropriate community resources and support services.
  • Provide health education and resource information in a culturally responsive manner.
  • Assist patients with applications, transportation, and other barriers to accessing care and services.
  • Follow up on referrals and outreach efforts to promote successful resource utilization.
  • Advocate for patients and support improved access to healthcare and community services.
  • Help patients navigate healthcare systems and communicate effectively with providers.
  • Collaborate with providers and care teams to develop care plans and coordinate patient support services. Qualifications
  • High school diploma or equivalent required.
  • Knowledge of community resources and social service programs.
  • Experience working directly with individuals, families, or community members in a support, healthcare, human services, or related setting.
  • Valid driver's license for work-related travel.
  • Mandatory Reporter certification or ability to obtain upon hire.
  • Strong verbal and written communication skills.
  • Strong interpersonal skills with the ability to build relationships with diverse populations.
  • Proficient computer skills and ability to document and navigate electronic systems.
