Caribou Medical Center is hiring a Swing Bed Program Coordinator (RN) Are you a compassionate RN with a passion for care coordination and patient advocacy? Why This Role Matters Make a real difference: Guide patients through critical care transitions Lead program growth: Help build and expand a growing Swing Bed program Collaborative environment: Work closely with nursing, case management, and providers Patient-focused impact: Improve safety, outcomes, and patient satisfaction What You'll Do Leadership & Coordination Manage Swing Bed admissions and discharges efficiently and without delays Coordinate patient reviews and ensure timely admission decisions Analyze data to identify opportunities for program growth and improvement Support nursing department goals and ensure alignment with patient care standards Care Coordination & Collaboration Partner with Case Management and Patient Navigators to increase referrals Integrate post-discharge needs into individualized patient care plans Serve as a key liaison between patients, families, and healthcare teams Coordinate with community resources to support safe transitions of care Patient Advocacy & Education Advocate for patients, families, and the facility Lead patient/family and interdisciplinary team meetings Provide education on chronic disease management and recovery Ensure patients understand discharge plans and follow-up care Clinical Support Step in as a primary nurse when needed during high census or acuity Participate in care planning to ensure goals are measurable and appropriate Monitor quality concerns and support patient safety initiatives Chronic Care & Transition Management Support chronic care management programs and patient outreach Conduct follow-ups via phone, in-person visits, or electronic communication Help patients set and achieve self-management goals Participate in ACO initiatives and care transition programs Program Growth & Quality Improvement Support marketing and community outreach for the Swing Bed program Participate in audits, quality improvement efforts, and performance reviews Identify and resolve barriers to program success and patient satisfaction What Makes You Successful You thrive in a fast-paced, high-volume environment You are a strong communicator and team collaborator You are passionate about patient advocacy and education You're driven to improve processes, outcomes, and patient experiences Please note duties, responsibilities, and activities of this position may change at any time with or without notice. Required Qualifications Registered Nurse (RN) with an active Idaho license (or compact license with Idaho licensure within 90 days) Associate Degree in Nursing (BSN preferred) Current BLS certification Knowledge of Medicare, Medicare Advantage, and insurance regulations Experience with electronic health records Strong clinical skills, including medication administration Preferred Qualifications Population health or care management experience Experience with care transitions or discharge planning Strong communication, organization, and problem-solving skills
