Care Management Navigator

The Patient Care Navigator is responsible for assisting Care Managers to provide outreach efforts to a group of patients from a patient list. The Patient Navigator will share knowledge and education to empower patients and improve their ability to navigate services and systems. This role will utilize the established model of care management. The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures, including complying with all Rush Customer Service Standards. This position will help to facilitate placement of the Behavioral Health population in the ED. Care Management Navigation

• Daily monitoring of a list of ER discharges, and referrals.
• Provide outreach for all patients due for follow up and new patient appointments according to policy and procedures.
• Provide all applicable patients with IMM and Moon forms as required by regulation
• Document all correspondence in electronic medical records as necessary.
• Liaison to multiple departments, agencies and provider offices.
• Coordinate transportation for all patients as needed

Position Requirements:

• High School degree or equivalent required Associates degree preferred
• Must have excellent oral communications in order to solicit information and communicate effectively with stakeholders
• Knowledge of Microsoft Office, EPIC, Careport, preferred
• Written, verbal and exceptional interpersonal communication skills. Bilingual preferred
• Prefer knowledge of medical terminology
