Athletic Trainer - Ortho Care Navigator Parkview Health - 3.7 Fort Wayne, IN Job Details Full-time 1 day ago Qualifications CPR Certification Certified Athletic Trainer BLS Certification CPR Full Job Description Summary Primary role is to assess patient needs in order to perform appropriate interventions to support the patient in the orthopedic service line continuum of care. Works collaboratively with various members of the health care team to provide guidance and support to patient to reduce obstacles of care, and to facilitate a clear path back to baseline. Is a creative problem-solver who serves as knowledgeable resource and coordinator between client, healthcare team, and available community resources. Assists patients with obtaining community assistance by referral and coordination with appropriate resources and scheduling and preparation for appointments. Helps to direct patients to appropriate support while maintaining necessary records and data. Promotes patient education. Builds and maintains relationships with patients and service line providers for purposes of improving quality of care. Understands the uniqueness of the individual and seeks the appropriate resources. Performs activities related to the provision of evaluation, training, and care of the injured athlete and other patients in accordance with the overall organizational goals of promoting Parkview Health. Education Must have a Bachelor's Degree or a Master's Degree in Athletic Training. Master's Degree preferred. Licensure/Certification Certification as an athletic trainer by the National Athletic Trainers Association Board of Certification required. Licensure or eligible for licensure by the Indiana Professional Licensing Board required. Valid driver's license preferred. Must have current CPR certification.
