Summary:
The Community Resource Navigator will serve as a primary point of contact for healthcare patients, as well as the communities we serve, in an ambulatory care setting. They will support individuals and families entering the healthcare system and assist with utilization and retention of benefits and community resources. They will also focus on helping eliminate barriers to all JPCHC services. This position will float to clinics as needed.
Essential Duties and Responsibilities :
Essential Duties include, but are not limited to the following: Work outreach events during traditional and non-traditional hours and locations Work collaboratively with all the different departments of the JPCHC organizations Meet with self-pay patients to discuss payment options and complete Sliding Fee Scale agreement Maintains a high level of confidentiality while interacting with patients and clients with empathy and respect Research and resolve patient or client issues, including consulting with providers, plan administrators, and insurance companies Educate members on issues related to using the healthcare system and their specific government assistance plan Maintain HIPAA standards and confidentiality of protected health information Follow up with clients/patients who request in-person government assistance Maintain Food Pantry at designated location, including pantry access, stocking, ordering, and cleaning. Follow through with Tele-Visit appointments (appointments where the consumer is at a separate location from where you are located) Assist patients with applications and follow-up for Medicaid programs and Marketplace coverage Assist patients with Medicare Process income information for the JPCHC Sliding Fee Scale applications Assist patients with applications for various Prescription Assistance Programs Provide community resources to the underserved, vulnerable populations, and patients Participate in outreach and education programs for the community, such as health fairs, screenings, and other events Assist with Social Determinants of Health where needed Assist with special projects and other duties as necessary and required Perform related duties as assigned
Qualifications :

Experience:
High School Diploma or Equivalent required. Bi-Lingual/Multi-Lingual a plus Two years of previous experience in outreach navigation preferred Customer service experience required Must be willing to travel 50% of the time to different site locations Valid Indiana Driver's License and proof of auto insurance required Certified Indiana Navigator certification required within 90 days of employment Certified Application Counselor certification required within 90 days of employment Community Health Worker certification required within 6 months of employment Computer Skills To perform this job successfully, an individual should have proficient computer skills, including Microsoft Office (specifically Outlook, Excel, Word, and PowerPoint) and Adobe Acrobat.
