Full Job Description Position Summary :
The Patient Navigator plays a key role in supporting patients during inpatient treatment and through discharge by coordinating care, addressing external stressors, and ensuring patients are set up for long-term recovery success. This role involves frequent interaction with patients, treatment teams, external service providers, and community partners.
Schedule:
Monday-Wednesday & Saturday 9:30a-3:30p
Essential Duties and Responsibilities :
Assess patients' external biopsychosocial needs (e.g., housing, financial, legal, medical, employment) and develop strategies to address them during and after treatment Facilitate care coordination, aftercare planning, and communication with outside agencies and services Manage documentation and paperwork related to FMLA, short-term disability, and court matters Maintain communication with patients' support systems, employers, and referral sources Keep thorough case notes and update patient records accurately and promptly Coordinate discharge planning and ensure continuity of care with community-based services Participate in weekly treatment team meetings to provide insight and updates on patient progress and discharge readiness Assist with census management by coordinating discharge schedules and communicating updates to both clinical and administrative teams
Qualifications :
CADC or TCADC certification with a Bachelor's degree (preferred) Minimum 1 year of experience working in a human service setting with adults or adolescents Strong commitment to a drug-free lifestyle and responsible alcohol use Personal interest or experience in substance use or behavioral health recovery strongly preferred Must meet all requirements outlined by state regulations
