Job Description

Job Summary:
As a Chronic Care Managers (CCM) you will assess provider instructions, patient health questions, and patient needs to develop and implement each qualifying patient a chronic care plan with goals that are patient specific. CCMs advise patients on health maintenance, education, and disease prevention with guidelines of scope of practice and by provider instruction.
Responsibilities:
Complete 20 minutes or more per month on each CCM patient. Maintain accurate detailed reports and records. Follow HIPAA regulations in all areas of contact with patients, providers, families, caregivers, and other healthcare or community health agencies. Record medical information given by patients, goals reached, questions, and vitals if given. Modify patient treatment plans as indicated by patient responses, conditions, and provider interactions. Consult and coordinate with other team members to assess, plan, implement, or evaluate patient care plans. Prepare patients for upcoming provider visits, remind of any upcoming specialist appointments, tests, procedures, and/or imaging. Review previous visits, current goals, and any changes that have occurred. Schedule procedures, tests, and appointments as needed.
Experience/Skills:
ERM experience preferred. Medical office experience preferred. Requires exceptional interpersonal and communication skills, with the ability to interact effectively with patients, the public, and fellow team members in a courteous, professional manner. Ability to write professional correspondence via letters or email. Highly developed attention to detail and organizational skills are critical. Ability to manage multiple tasks and meet deadlines. Ability to cross train in other functions, if necessary. Knowledge of medical insurance and billing is preferred. Working knowledge of basic communication software, including MS Office.
Education:
High school diploma or equivalent required.

$16.00 - $18.00 per hour
