Job Description
Summary The Clinical Liaison is responsible for building and maintaining a professional relationship with key referral sources with LA Extended Care Hospital of Lafayette referring entities. Through the development of relationships, they generate patient referrals from their assigned accounts. They are responsible for meeting their volume expectations for referrals and admissions. The Liaison performs relationship development with referral sources that both directly refers and influences patient referrals. These contacts are primarily planned and unplanned face to face contacts and they require the skill to expand and maintain strong relationships throughout a referring organization. The Liaison develops strategic plans to effectively manage their accounts and to expand into new accounts as assigned. The liaison performs clinical assessments to determine the LTACH appropriateness of all patient referrals and meets with patients and their families to educate about our hospitals, services and programs. The Clinical Liaison is actively involved in department activities, transdisciplinary team activities, and LA Extended Care Hospital of Lafayette activities to ensure individualized, patient-centered health care populations admitted to LA Extended Care Hospital of Lafayette . Responsibilities Develop and maintain strong relationships with hospital partners and referral sources. Facilitate smooth transitions for patients from acute care to home health services. Conduct outreach to educate healthcare providers about PHG's services. Collaborate with clinical teams to ensure high-quality patient care and satisfaction. Monitor patient outcomes and address any concerns or issues that arise during the transition process. Provide training and support to internal staff on best practices for patient engagement. Analyze market trends and identify opportunities for growth within the community. Participate in community events to promote PHG's mission and services. Qualifications Proven experience in healthcare or clinical liaison roles, preferably in long term acute care settings. Strong communication skills with an emphasis on relationship-building and customer service. Demonstrated ability in account management and outside referral strategies. Organizational skills to manage multiple accounts effectively. Negotiation skills to facilitate agreements with referral sources. Familiarity with healthcare regulations and practices is a plus.
