BHN is currently seeking a Care Navigator to join our Integration team, supporting individuals with behavioral health and substance use challenges as they re-enter the community from justice and medical systems. Your work will help ensure successful community re-entry, treatment engagement, and long-term stability. Meet with individuals in-person or via telehealth at jails, hospitals, and treatment facilities to support discharge planning and community transitionAct as the primary point of contact for a caseload of individuals, ensuring their needs are identified and metComplete assessments, support plans, safety plans, and disenrollment planning under the guidance of the Clinical SupervisorConnect individuals with critical resources such as housing, food assistance, vocational supports, medical care, and recovery servicesCoordinate with probation officers, parole officers, courts, correctional staff, and community providers to support careProvide crisis management with support from Clinical Supervisors and the Emergency Services ProgramEducate individuals on coping strategies, resource access, and skills for greater independence and self-sufficiencyMaintain professional documentation in line with program and funder requirements Who You Are High School Diploma or GED required; Bachelor's degree or Community Health Worker certification preferred1-2 years of experience in behavioral health, substance use, corrections, or related human services field requiredKnowledge of community resources and strong ability to connect individuals to supportsExperience working with justice-involved populations and/or recovery from behavioral health/substance use preferredBilingual in Spanish/English strongly preferredStrong communication, organizational, and problem-solving skillsValid driver's license and reliable vehicle required We Hire for Purpose!
