Case Manager Make a Difference in Your Community We are seeking a compassionate, motivated, and organized Case Manager to join our team. In this role, you will provide trauma-informed, person-centered support to individuals and families by connecting them with critical services and resources that promote independence, recovery, and community inclusion. What Youll Do Develop and coordinate individualized service plans (IPOS)Advocate for consumers and connect them to healthcare, housing, education, employment, and community resourcesSupport consumers through crisis planning, transitions, and recovery goalsCoordinate care with providers, families, and support systemsMonitor services and ensure consumers receive the support they need to thrive
