Job Description

Position Purpose:
In partnership with hospital staff, utilizing Project ASSERT protocols, Wellness Advocates will work with identified patients involved in substance use to offer screening, brief intervention, and referral to treatment as needed.
Principal Responsibilities:
Communicates with hospital team to receive referrals regarding patients involved with substance use. Completes a confidential face-to-face Brief Negotiated Intervention (BNI) with patients to screen for substance use issues and provide appropriate resources as applicable. Discusses the process of change, uncovers barriers to care, and develops a plan of change with the patient. Provides education on available resources which address the patient's identified needs. Demonstrates a knowledge of community resources and advocates for/assists the patient to connect with outside agencies. This includes a base knowledge of how funding surrounding the resources works as well. Models healthy recovery and demonstrates healthy coping skills with both patients and hospital staff. Completes post-discharge follow-up calls with patients to provide encouragement, re-engagement, and any additional resources that the patient identifies 7, 30, and 60 days following initial interaction. Accurately and timely tracks all encounters in multiple databases for agency, hospital, and funding sources. Builds and maintains respectful and professional relationships with hospital staff and peers. Attends team, agency, and hospital meetings and trainings as required. Other duties as assigned.
Experience and Skills:
High School or GED required with 2-3 years relevant work/life experience. Associates degree in a medical, human services, psychology, or related field preferred. Be actively engaged in a recovery process or program and able to clearly articulate the practices, supports, and accountability structures the personally use to sustain their recovery, Maintain personal conduct consistent with recovery-oriented peer practice, Maintain sustained abstinence from alcohol, marijuana, and other drug use including misuse of prescribed or unprescribed medication, and Demonstrate a minimum of three (3) consecutive years of continuous recovery. Strong working knowledge of recovery principles. Project ASSERT training completed within the first 2 months of employment CCAR completion or CPRC certification within the first 6 months of employment Moderate computer skills. Demonstrate a working knowledge of Microsoft Office applications and virtual meeting platforms.
