Job Description
Responsibilities At St. Louis Behavioral Medicine Institute (SLBMI), the IOP/PHP Care Coordinator is responsible for the administrative support related to the success of all of the IOP/PHP Programs at SLBMI. This position provides oversight of the admission process, clinical assignment, chart audit, and reporting that supports the IOP/PHP Program as it relates to adherence to accreditation standards for providing safe, high-quality care. The IOP/PHP Care Coordinator works alongside the IOP Care Navigator to assist with improving the patient experience, requirements for admission and discharge processes. SLBMI is a freestanding outpatient facility that serves patients in locally in the St. Louis region as well as nationally through its telehealth services. It is known for its speciality behavioral health care, providing services for complicated behavioral health conditions by its team of 75+ licensed providers. This position is primarily housed out of its Macklind Location, with assignment 1-2 days per week at its Chesterfield location. This is an in-person position.
Shift:
Monday - Friday, day-shift. Essential Job Duties/Responsibilities Oversee and improve the patient entry, census, and discharge to regularly meet/exceed budgeted census expectations. This would include improving communication with referral sources both prior to admission and after discharge with the goal of building referral relationships to the IOP. Weekly reporting of this information to the Director, CEO, and CFO Assists with initial and ongoing Insurance Authorizations for patients in IOP/PHP as needed Maintains communication with patient from the point of being identified as an
IOP / PHP
patient including communication for next steps, admission dates, and answers any questions regarding the pre-admission process. Performance improvement monitoring and administration to include data tracking and reporting for each IOP/PHP from initial evaluation to discharge, symptom surveys pre, mid, discharge, and post-treatment, and patient satisfaction surveys to IOP/PHP patients. Active communication with the IOP/PHP treatment teams, including attendance at IOP/PHP Program meetings and ongoing communication with the treatment team. Tracks attendance based on communication with treatment team and EHR records. Monitors and assists IOP/PHP treatment teams in updating treatment plans during weekly staffing meetings to align with identified compliance standards. Oversees documentation completeness, including treatment plans, quality improvement measures, discharge plans, and IOP-related documentation. Communicates incomplete documentation to IOP Director and relevant clinical staff throughout treatment. Coordinates with the IOP/PHP treatment teams to ensure the implementation of discharge planning at the start of admission and during treatment as well as the completion of discharge summaries and transition at the point of discharge. Acts as a liaison for outpatient treatment providers (both internal and external providers) to communicate and coordinate care at the point of admission, updates to treatment plan progress, and discharge planning throughout a patient's participation in IOP/PHP. Facilitates internal and external referrals based on IOP/PHP after care and discharge plan (i.e. psychiatry, dietitian, or outpatient services). Attend all staff meetings relevant to IOP and the coordination of IOP patient care.
