Position:
Chronic Care Management Specialist (Care Navigator)
Location:
Cary, NC (on-site position working directly from outpatient care clinic)
Schedule:
Monday-Friday and weekends if needed to close out month end process!

Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) are revolutionizing how we care for patients with long-term health conditions. As a Chronic Care Management Specialist at Axil Health, you'll be the vital link between our patients and their healthcare team. Your work will directly contribute to: Reducing hospital readmissions Improving medication adherence Enhancing patients' quality of life Empowering patients to take control of their health Providing peace of mind to patients and their families
What You'll Do:
Be a health hero to over 500 patients across North Carolina Build lasting relationships through regular check-ins and support calls Use state-of-the-art remote monitoring tools to track patients' health in real-time Collaborate with our clinical pharmacists to optimize patient care plans Educate and motivate patients to achieve their health goals Be the friendly voice and listening ear that patients look forward to hearing from
Who We're Looking For:
Certified Medical Assistants (MAs) who are passionate about patient care Empathetic individuals with excellent communication skills Tech-savvy professionals comfortable with digital health platforms Detail-oriented team players who can juggle multiple priorities Healthcare enthusiasts eager to learn and grow in an innovative environment Required Qualifications (one of the following): Nationally recognized Medical Assistant (MA) certification LPN License Why You'll Love Working at
