Job Description:
The Recovery Navigator position is responsible for the coordination of services and needs identified by patients as well as short-term case management of individuals as needed. The primary responsibility of the Recovery Navigator is to work with Substance Abuse Disorder (SUD) patients and community members to identify, locate, and obtain health, social, and community services. Recovery Navigator will work closely with other members of the healthcare team across the organization to help facilitate community connections, provide warm hand-off to community agencies and to provide ongoing support for recovery. Responsible for the facilitation of educational workshops in the community.
Qualifications:
Associate degree in human services or related degree with 3 years experience working with individuals with Substance Abuse disorders. Strong customer service skills. Strong communication and time management skills. Cross-cultural skills and experience with culturally diverse populations.
Preferred:
Bachelors degree in human services or related degree with at least 1 year of experience working with individuals with Substance Abuse disorders.
Experience:
A minimum of three years experience working with community members and community organizations to promote health and address socio-economic barriers to health. Experience with the addiction recovery process. Understanding of medication-based recovery and practices. Familiarity with local support services and resources. Computer skills, including knowledge and proficiency of Microsoft Office. Ability to relate well with all segments of the public. Perks +
