SSVF Healthcare Navigator Blue Valley Community Action (BVCA)
Full-Time:
36 hr work week | Non-Exempt | Office located in Omaha NE Serve Those Who Served Blue Valley Community Action is seeking a dedicated SSVF Healthcare Navigator to support veterans in accessing healthcare, achieving wellness goals, and maintaining housing stability. In this role, you will advocate for veterans, coordinate care between the VA and community providers, and provide case management using a Housing First, veteran-centered approach. What You'll Do Help veterans identify and achieve healthcare goals Serve as a liaison between veterans and the VA healthcare system Assist with obtaining safe and affordable housing Provide case management and care coordination Assist with medical and mental health appointments Reduce barriers to care and promote treatment adherence Complete required documentation in HMIS and maintain compliance Collaborate with community partners and treatment teams Transport veterans to appointments as needed Qualifications Bachelor's degree or above, preferably in human services. Must have experience in case managment , preferably in the healthcare field.
