ROLE DESCRIPTION
Position Summary The Oncology Lay Patient Navigator works with the Oncology Nurse Navigators to provide a personalized level of support to help patients, families, and caregivers - from screening to survivorship - understand their care. In collaboration with the multidisciplinary team they are responsible for coordinating timely access to quality and comprehensive health and psychosocial services throughout the continuum of care. They have knowledge of all AtlantiCare services, programs, providers and resources. The Oncology Lay Patient Navigator is a trained layperson who can help make sure that cancer care isnt pushed aside for patients juggling these challenging circumstances. They can help guide patients through the often-complicated health care system with the resources they need to get care. They demonstrate an ability to help patients and their families in understanding and coping with the strong emotions that accompany a cancer diagnosis. They remain a constant caregiver and overseer of the patients and families educational, emotional and social needs. The Oncology Lay Patient Navigator is a primary point of contact for the patient and works with other members of the care team to coordinate care for the patient. This critical person on the multidisciplinary team provides an important perspective on the logistical, structural, and social needs of the patient, as well as cultural considerations, patient values, and care preferences. The Oncology Lay Patient Navigator provides exceptional customer service to all. They demonstrate strong problem-solving skills to appropriately respond to complex situations while exhibiting high level time management skills and flexibility. The Oncology Lay Patient Navigator answers all calls in a friendly, timely, courteous and professional manner while dealing with each person compassionately, and showing genuine concern for each situation. The Oncology Lay Patient Navigator responds to email requests and internet inquiries within specified timeframes concisely and accurately in a professional and proper manner. This position communicates and collaborates with the Oncology Nurse Navigator team members and other hospital personnel to provide quality customer service and smooth transitions. Supports organizational goals by providing quality customer service, participating in performance improvement efforts, and demonstrating a commitment to teamwork and cooperation.
Qualifications Education:
High School Diploma or equivalent required. Graduate of a Medical Assistant program required upon hire or transfer.
License/Certification:
Certified Medical Assistant required upon hire or transfer. American Heart Association BLS required upon hire or transfer.
Experience:
A minimum of 2 - 5 years of health care experience including medical terminology required. Managed care experience is strongly preferred. Additionally, a minimum of 2-5 years of customer service or community/public interaction required. Excellent computer skills and good knowledge of EMRs. Performance Expectations Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position. Work Environment Potential for exposure to the hazards and risk of the hospital environment, blood borne pathogens, including exposure to infectious disease, hazardous substances, and potential injury. This position requires frequent walking and standing. The essential functions for this position are listed on the Assessment and Evaluation Tool. Reporting relationship This position reports to department leadership and may supervise incumbents as assigned.
