Pay Transparency:
The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey. The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
Job Overview:
The role will assist in efforts to collect information on family social needs/barriers/issues at Children's Specialized Hospital. The Patient Navigator will review patient data and identify appropriate ways to identify and assist families in need. The Family Navigator will maintain strict confidentiality of the information collected of patients and their families.
Qualifications:
Required:
Excellent communication skills and a team player Must be self-motivated and well organized
Preferred:
Ability to incorporate and model CSH's core values Familiarity with computer software packages (Microsoft Office - particularly Word and Excel) Previous hospital or physician's office experience
Scheduling Requirements:
Day shift, 9:00am-12:00pm (flexible) Monday through Friday, no weekends Per Diem, up to 15 hours per week
Essential Functions:
Collects information regarding social needs/barriers/issues directly from families Effectively engages and establishes helping relationships Identifies and provides resources to families while maintaining patient confidentiality Reviews patient data and identifies appropriate families to contact Acts as an information and emotional resource to patients and families Provide general information as requested, and direct families appropriately Communicates social needs/barriers/issues to a multidisciplinary team at the hospital in an appropriate manner Monitors and obtains accurate information for data collection, and assist with data analysis Other tasks assigned by management
