Job Description

Job Summary We are seeking a dedicated and compassionate PRN Licensed Medical Social Worker (MSW) to join our hospice care team. In this vital role, you will provide comprehensive psychosocial support to patients and their families, ensuring they receive personalized, empathetic care during challenging times. Your expertise will help facilitate emotional well-being, assist with complex decision-making, and promote quality of life through tailored counseling and case management. This flexible position offers the opportunity to make a meaningful difference in patients' lives while working within a collaborative, patient-centered environment. Responsibilities Conduct thorough patient assessments to evaluate psychosocial needs, including mental health status, family dynamics, and social support systems. Provide individual and group counseling sessions utilizing evidence-based approaches such as cognitive behavioral therapy, motivational interviewing, and behavioral health techniques. Assist patients and families with advance care planning, grief support, and coping strategies during end-of-life care. Coordinate with interdisciplinary teams to develop and implement personalized care plans that address emotional, social, and spiritual needs. Maintain accurate medical documentation in compliance with HIPAA regulations and organizational policies, including detailed case notes and progress reports. Collect relevant data for clinical research initiatives aimed at improving hospice services and patient outcomes. Facilitate crisis intervention when necessary, providing immediate support during emotional or behavioral emergencies. Qualifications Valid state licensure as a Licensed Master of Social Work (LMSW) or equivalent. Experience working in hospice care or palliative settings is highly preferred. Strong knowledge of medical terminology, medical records management, and healthcare documentation standards. Proficiency with Electronic Medical Record (EMR) systems and familiarity with CPT coding for billing purposes. Excellent communication skills with the ability to perform comprehensive patient assessments and deliver compassionate clinical counseling. Ability to work independently in a PRN capacity while maintaining flexibility to support team needs across various settings including home & community care environments. Must be a licensed driver with an insured automobile in good working order.
