Responsibilities/Job Function:
Remote Patient Monitoring Ensure a seamless, complete and effective enrollment process for all prospective patients into the remote monitoring program 5 days week in clinic locations Responsible for monitoring vital signs remotely and calling the patient to assess need for intervention by their healthcare provider or whether further monitoring is appropriate. Building relationships with patients for engagement, support and communication. Remote patient onboarding and providing education for monitoring devices, mobile app, and platform for the RPM program. Remotely monitoring enrolled patients through daily review of electronic patient data (adherence, physiological data, patient messages, etc.). Providing education and other tools to support improved patient outcomes. Communication between the provider and other members of the care team, as needed. Initiates phone contact with eligible Medicare patients in an attempt to enroll them in
CCM/RPM/RTM
programs on behalf of physician practice. Completely and accurately educates eligible patients on the benefits of
CCM/RPM/RTM
programs. Obtains consent from patients electing to opt-in to the programs. Maintain access to, and security of, patient data and information. Troubleshoot devices and report malfunctions. Answer routine inquiries and relay information and messages to and from appropriate health professionals. Chronic Care Management Conduct scheduled monthly telephone check-ins (minimum 20 minutes) with assigned patients to assess health status, monitor symptoms, and identify changes. Review medication lists with patients, confirm compliance, identify potential barriers to adherence, and coordinate refills with pharmacies and providers. Act as a central point of contact to coordinate referrals to specialists and schedule follow-up appointments. Provide education on chronic condition self-management, lifestyle modifications, and proper use of remote monitoring devices. Maintain accurate, real-time documentation of all patient interactions to ensure compliance with CMS guidelines and enable accurate billing for CCM services. Assist in developing, updating, and documenting comprehensive, patient-centered care plans in the Electronic Health Record (EHR) under provider supervision. Personal Skills/Attributes/Qualifications Certified Medical Assistants/LPN/LVN/CNA with CMT - certification required. 2- 5 years of related professional or healthcare medical experience. Spanish language speaking and writing skills are required. Candidates will be required to pass a bilingual (Spanish and English) proficiency test during the recruitment process. This is an ONSITE position. Strong analytical and problem-solving skills. Capable of working in a fast paced work environment with a positive attitude. Must be a self-starter able to work independently and with teams. Proficient in using Word, Excel, and PowerPoint. Excellent oral and written communication skills. Proficient in managing multiple tasks at the same time. Tech-savvy. Experience in using EHR systems (Epic, Athena, eClinicalWorks, etc.). Strong telephone etiquette and active listening skills for remote patient engagement. Ability to manage a panel of 200-600 patients. Understanding of chronic disease management, terminology, and HIPAA regulations. General Duties Attend customer meetings. Will be required to interact with customers and patients in multiple time zones. Other duties as assigned.
Pay:
$12.00 - $16.00 per hour
