Job Overview We are seeking a dedicated and proactive Social Services Coordinator (Discharge Planner) to join our dynamic healthcare team. In this vital role, you will coordinate patient discharge planning and post-acute care arrangements, ensuring seamless transitions from inpatient settings to outpatient or community-based services. Your expertise will help optimize patient outcomes, enhance care continuity, and support patients and their families through complex healthcare journeys. This position offers an exciting opportunity to make a meaningful difference in patient care while working within a fast-paced, multidisciplinary environment. Responsibilities Collaborate with medical teams to assess patient needs through comprehensive evaluations, including medical records review, diagnostic assessments, and physiological understanding. Develop individualized discharge plans that incorporate patient preferences, clinical requirements, and available community resources. Coordinate with inpatient units, outpatient clinics, post-acute care providers, and community agencies to arrange appropriate services such as rehabilitation, home health, or behavioral health support. Facilitate communication between patients, families, healthcare providers, and insurance companies to ensure clarity around treatment plans and coverage options. Maintain detailed documentation of patient assessments, discharge plans, case management activities, and utilization management records in compliance with HIPAA regulations. Conduct intake assessments for new patients requiring social work or mental health counseling services and evaluate eligibility for various programs. Support patients in navigating complex healthcare systems by providing education on medical terminology, hospital procedures, and post-discharge care instructions. Requirements Proven experience working in hospital settings with exposure to Level I Trauma Centers or Level II Trauma Centers is highly preferred. Strong background in acute care environments such as ICU units, emergency medicine departments, or hospital medicine wards. Familiarity with pediatric care settings including PICU (Pediatric Intensive Care Unit), as well as outpatient clinics and post-acute care facilities. Knowledge of behavioral health issues, mental health counseling techniques, addiction counseling strategies, and inpatient/outpatient case management practices. Experience with hospital discharge planning processes involving diagnostic evaluation and utilization management protocols. Excellent understanding of medical terminology, physiology concepts related to patient assessment, and medical records management. Ability to work effectively within managed care frameworks while maintaining a focus on patient service and quality care delivery. Strong communication skills for collaborating across multidisciplinary teams and engaging with diverse patient populations.
