Full Job Description FLSA Status:
Non-Exempt Starting Rate:
$27.47 Full-Time 35hrs per week.
SUMMARY OF DUTIES
The Family Engagement Team Leader functions as the leader of a multidisciplinary team of professionals who deliver services to consumers of BestSelf Behavioral Health's Family Engagement Team. Services provided include outreach, engagement, assessment, mediation, service linkage and crisis interventions to transition aged youth who are homeless or at-risk of homelessness and their families of choice. Supervises the day to day operations of the program, provides direct supervision to staff, facilitates weekly staff meetings and provides on-going staff trainings. Assists Program Director with data collection and reporting to ensure compliance with funding requirements. Maintains a fundamental understanding of transition aged youth, mental illness and substance abuse disorders; their related symptomatology and rehabilitation. Must be able to develop and maintain positive collaborations in the community. Provide outreach services to effectively assist in targeted outcomes. Operates at a high level of independence and responsibility under supervision of the Homeless Services Program Director.
RESPONSIBILITIES
Initiates and maintains effective collaborations with all systems involved in providing services to the homeless youth Advocates for the unique service needs of homeless youth including those who possess no linkage to treatment and non-compliance with all systems of care Compiles and submits monthly reports in accordance with state, county and federal regulations Completes regular supervision, training and performance evaluation of staff. Performs outreach, engagement, intake, assessment, service planning, service linkage, case follow up and discharge planning to a small client caseload. Works with client to develop and enhance independent living skills and effective interpersonal communications, and support networks in order to achieve client goals and objectives. Intervenes in crisis or pre-crisis situations to limit and contain problems Transports clients and provide advocacy, when needed. May work with individuals harmful to themselves or others. Must be able to work flexible hours including some evenings and weekends as needed Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.
REQUIRED KNOWLEDGE AND SKILLS
Must possess excellent communication skills, both written and verbal. Must possess excellent computer skills. Must possess excellent leadership, organizational and problem-solving skills. Must possess excellent crisis management skills Must be able to maintain BestSelf HIPAA confidentiality guidelines. Able to deal effectively with clients, staff, providers and general public. Able to maintain appropriate personal and professional boundaries. Flexibility to deal effectively with a variety of people, situations, problems and changes. Requires knowledge of mental illness diagnosis, symptomatology and engagement. Possesses familiarity with community resources, including entitlement programs, medical, financial, legal services, supported housing and section 8 programs, mental health and addiction services.
QUALIFICATIONS
Bachelor's degree in a related field plus two (2) years full time paid human service experience in an approved mental health or social service agency and program management experience OR Associates degree or High School diploma and five (5) years full time paid human service experience in an approved mental health or social service agency and program management experience. AND A valid NYS drivers' license. Automobile in good working order. Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire. Must maintain Active NYS Defensive Driving class every 3 years, upon expiration. If driving one's own vehicle, must maintain valid inspection and automobile insurance.
