$24.73/hour The Opportunity Make a real-world impact as a Patient Navigator. In this high-impact role, you will provide critical sexual health testing, evidence-based education, and seamless care linkage to reduce HIV, HCV, and STI transmission across Long Island. You will blend community outreach with vital harm reduction work, including overdose recognition, Narcan response training, and substance use support. Responsibilities Collaborate with internal FCA teams (THRIVE Recovery Centers, Clinical Services, Sherpa) to engage individuals at high risk for HIV, HCV, and STIs. Conduct field outreach, build strategic community partnerships, and leverage virtual platforms to connect diverse populations with prevention services. Deliver rapid testing for HIV, HCV, and syphilis, collect clinical specimens, and facilitate external referrals for comprehensive STI screening. Ensure barrier-free, rapid linkage to medical treatment, preventative care, and vital PrEP/PEP resources for positive or high-risk clients. Perform trauma-informed risk assessments and build custom risk-reduction plans using motivational interviewing and stages of change models. Integrate life-saving harm reduction and overdose prevention education, including hands-on Narcan training, into everyday client interactions. Distribute public health prevention tools and safer sex supplies while providing clear instructions on proper usage. Facilitate engaging educational workshops at FCA facilities and local community venues to boost regional health awareness. Maintain timely, compliant documentation of all client services within the Electronic Medical Record (EMR) system. Submit precise, high-integrity service data to key funding partners like OASAS and the New York State Department of Health. Engage actively in team supervisions, cross-functional staff meetings, and continuous quality improvement projects. Coordinate directly with healthcare providers and external stakeholders to deliver continuous, client-centered care. Handle additional supportive duties as assigned by the Program Director. Education:
High School Diploma or Equivalent Certificates of Licenses:
N/A Experience:
At least 6 months of related experience in healthcare, human services, or social work. Equivalent training or relevant lived experience in
HIV/AIDS
prevention, PrEP/PEP navigation, harm reduction, or substance use support will also be highly valued.
