Job Description

Job Summary :
This position helps to "navigate" patients through the health care system, including providers' offices, hospitals, insurance and payment systems, patient-support organizations and other components of the health care system. In addition, the Patient Navigator helps to identify barriers that may be preventing patients from receiving timely and high quality healthcare treatment and working to find solutions to those barriers.
Responsibilities:
Assists logistics coordinator with network wide escalations of discharge barriers Assists with expediting barriers to progression of care Assists in overcoming logistical barriers to scheduled diagnosis and treatment appointments. Provides outreach to patients to help them understand the importance of preventive services, based on the age and condition(s). Helps patients to understand recommended follow-up Contacts patients who are "at risk" for missing appointments. Provides educational tools as indicated. Facilitates access to available resources for obtaining insurance coverage and/or sliding fee benefits. Assist to ensure that appropriate information is available in the patient's medical records during scheduled appointments. Facilitate linkages to follow-up services. Coaches patients to become advocates for their own care. II. Build awareness of patient navigator services among the health care team by building professional relationships. With team members, providing information about patient navigator service and maintaining communication to identify patients who are "at risk" for barriers to care. Communicate with providers about unique patient needs, (i.e., cultural). Other duties as assigned.
Qualifications/Requirements:
Experience:
Two years of experience in healthcare, required. Knowledge about health care systems, the breadth and accessibility of community resources and working knowledge of HIPAA regulations, preferred.
Education:
High school diploma or equivalent required. Associate's Degree, preferred. Licenses /
