Job Description

Role Description The Care Navigator (RN/LPN) Key responsibilities include coordinating referrals, scheduling follow-up visits, maintaining regular patient contact, monitoring progress, and addressing barriers to care. The Care Navigator collaborates closely with physicians, nurses, social workers, and other healthcare professionals to facilitate smooth transitions of care and effective communication. Additional responsibilities include documenting patient interactions, tracking outcomes, educating patients about available healthcare and community resources, and supporting initiatives that enhance access to care, patient engagement, and overall patient satisfaction. Qualifications Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the State of Ohio required. Strong patient advocacy and customer service skills, with the ability to build trust, actively listen, and communicate effectively with patients, families, and healthcare teams. Excellent organizational and coordination skills to manage multiple cases, appointments, referrals, and care plans efficiently. Proficiency in electronic health records (EHR), standard computer applications, and documentation practices. Knowledge of healthcare systems, care coordination, and community resources; experience in a healthcare, clinical, or social services setting is preferred. Ability to work effectively in a hybrid environment, balancing independent responsibilities with team collaboration. Strong verbal and written communication skills, including the ability to explain complex information in a clear and understandable manner. High school diploma or equivalent required; an associate's or bachelor's degree in a health-related field, social work, or a related discipline is preferred.
